Section 1

Scope of This Privacy Policy

This Privacy Policy explains how Elite Performance Institute (“EPI,” “we,” “us,” or “our”) collects, uses, discloses, and protects personal information when you visit or interact with elite-performance-institute.com, contact our clinic, create an online account, purchase a digital product, or otherwise use our website and online services.

This website privacy policy is separate from our HIPAA Notice of Privacy Practices. If you become a patient of Elite Performance Institute, protected health information (“PHI”) created, received, or maintained in connection with your healthcare may be governed by HIPAA and other applicable health-privacy laws. Please review our HIPAA Notice of Privacy Practices.

General website forms, email, and ordinary website communications are not intended for emergencies. Please do not submit highly sensitive medical information through a general website form unless we specifically instruct you to do so through an appropriate method.

Section 2

Information We Collect

Information You Provide

We may collect your name, email address, telephone number, mailing or billing address, messages you send us, appointment or service inquiries, customer-support communications, and other information you choose to provide.

Account & Purchase Information

If you create a member account or purchase a digital program, we may collect account credentials, purchase history, product access, transaction status, and information needed to provide the product or manage your account.

Website & Device Information

We and our service providers may automatically receive information such as IP address, browser type, device type, operating system, referring pages, pages viewed, approximate location derived from IP address, timestamps, and interactions with the site.

Payment Information

Online payments may be processed by Stripe. Payment card details are generally submitted through Stripe’s payment infrastructure. We do not intentionally store complete payment-card numbers on our own website.

Section 3

How We Use Information

We may use personal information to operate, maintain, secure, and improve our website and services; respond to questions and requests; provide clinic and service information; administer member accounts; process purchases and refunds; deliver digital products; provide customer support; prevent fraud and misuse; comply with legal, tax, accounting, and regulatory obligations; and enforce our policies and agreements.

If you affirmatively subscribe to marketing or educational communications, we may use your contact information to send those communications. You may unsubscribe from non-transactional marketing emails using the unsubscribe link in the message or by contacting us.

Information associated with actual patient care may also be used as described in our HIPAA Notice of Privacy Practices and applicable patient forms.

Section 4

Cookies, Analytics & Embedded Content

Our website may use cookies, pixels, local storage, and similar technologies. Some are necessary for website security, login sessions, member access, checkout, or other requested functionality. Others may help us understand how visitors use the website and improve performance.

We may use Google Analytics and Google Tag Manager to measure website traffic and interactions. Google and similar providers may receive device identifiers, IP-derived information, page activity, and related usage data according to their own terms and privacy practices.

Pages may contain embedded content from third parties, including YouTube videos. When you load or interact with embedded content, the third party may receive information about your device and browsing activity and may place or access cookies or similar technologies.

You can control cookies through your browser and, where available, through website privacy or cookie controls. Blocking certain cookies may affect website functionality. Where applicable law requires consent before non-essential cookies or tracking technologies are used, we intend to honor those requirements through appropriate consent controls.

Section 5

How We Disclose Information

We may disclose personal information to service providers and vendors that help us operate the website or provide requested services, such as website hosting and security providers, WordPress-related services, membership or course software, payment processors, analytics providers, email or communication providers, and other technology vendors.

We may also disclose information when required by law; to respond to lawful legal process; to protect the rights, safety, property, or security of EPI, our users, or others; in connection with a business reorganization or transfer; or with your direction or consent.

We do not sell personal information for monetary consideration. We do not knowingly disclose patient PHI for advertising in a manner prohibited by HIPAA or other applicable law.

Section 6

Payments & Member Accounts

Stripe

We may use Stripe to process online payments, refunds, fraud checks, and related transaction services. Stripe may collect and process transaction data such as your name, contact information, billing information, payment method information, transaction amount, and purchase status under Stripe’s own privacy practices.

MemberPress / WordPress

If we offer protected digital programs or member content, account and access information may be stored within our WordPress website and managed through membership software such as MemberPress. We use this information to authenticate users, control access, and administer purchases and subscriptions.

Section 7

Security & Data Retention

We use reasonable administrative, technical, and organizational safeguards designed to protect personal information against unauthorized access, loss, misuse, alteration, or disclosure. No website, database, or transmission method can be guaranteed to be completely secure.

We retain personal information for as long as reasonably necessary for the purposes described in this policy, including providing services, maintaining business and transaction records, complying with legal and tax obligations, resolving disputes, preventing fraud, and enforcing agreements. Different categories of information may be retained for different periods.

Patient medical records and PHI are retained and handled according to applicable healthcare laws, professional requirements, and our clinical record-retention practices rather than this general website retention statement alone.

Section 8

Your Choices & Privacy Rights

Depending on where you live and the laws that apply, you may have rights regarding your personal information, which may include requesting access, correction, deletion, restriction, portability, or information about how your data is used or disclosed. Some rights are subject to exceptions or verification requirements.

You may contact us at info@elite-performance-institute.com or 331-472-4326 to make a privacy request. We may need to verify your identity before completing certain requests.

You can opt out of non-transactional marketing emails using the unsubscribe link in the email. You may also use browser or website controls to manage cookies where available. These website privacy rights are separate from the rights you may have regarding PHI under our HIPAA Notice.

Section 9

Children

Our website contains health, exercise, and sports information that may be viewed by families and youth athletes, but we do not knowingly use the website to solicit personal information directly from children under 13 without appropriate authorization. If you believe a child has provided personal information through the website inappropriately, please contact us.

Purchases and contractual account registrations should be completed by an adult or by a parent or legal guardian where required.

Section 10

International Visitors

Elite Performance Institute is located in Illinois, United States. If you access the website from another country, your information may be processed in the United States or in other locations where our service providers operate. Those locations may have data-protection laws that differ from the laws in your country.

Where applicable law grants you additional privacy rights or requires specific consent or transfer protections, we will apply those requirements to the extent they apply to our processing.

Section 11

Third-Party Websites & Services

Our website may link to third-party websites or services, such as payment processors, social media platforms, video platforms, maps, review sites, or other resources. Their privacy practices are governed by their own policies, and EPI is not responsible for the privacy practices of third-party websites that we do not control.

Section 12

Changes to This Policy & Contact

We may update this Privacy Policy periodically to reflect changes in our website, services, vendors, or legal obligations. The “Last Updated” date on this page identifies the most recent revision. Material changes may also be communicated through the website or by other appropriate means.
